1. Our Commitment to Your Privacy

Leticia Osei is considered a Health Information Custodian (HIC) under PHIPA and is responsible for
protecting your personal health information in accordance with Ontario privacy legislation.

2. What Is Personal Health Information (PHI)?

PHI includes identifying information such as your name, date of birth, contact info, health history, and session
notes, billing details, and communication records.

3. Collection and Use of Your Information

We collect only the information necessary to provide care, manage appointments, maintain records, fulfill
legal obligations, and ensure quality care. Consent is obtained unless disclosure is required by law. 

4. Safeguarding Your Information

We use physical (locked storage), technological (Sessions Health, encrypted email), and administrative
(limited access) safeguards. Records are stored for at least 10 years or until a minor turns 28

5. Your Rights Under PHIPA

You may access, request corrections, limit consent, and file complaints with the IPC.

6. Disclosure Without Consent

We may disclose PHI without consent in cases involving harm risk, legal requirements, or professional
regulation.

7. Third-Party Service Providers

We use Sessions Health, a PHIPA-compliant platform for secure communication, documentation, and billing.

8. Use of Our Website

Non-identifying data may be collected via cookies. Contact details submitted are used only to respond to inquiries.

Terms of Service

1. Scope of Services

Leticia Osei Psychotherapy provides individual psychotherapy services to adults in Ontario, delivered by a Registered Psychotherapist regulated by the College of Registered Psychotherapists of Ontario (CRPO). 

2. Eligibility

Clients must be 18 years or older and located in Ontario unless otherwise agreed. Emergency services are not provided. 

3. Appointments and Cancellations

 Appointments are 50 minutes. Cancellations must be made with at least 24 hours’ notice. Late cancellations or missed sessions may be charged the full fee. 

4. Fees and Payments 

Fees are discussed before service. Payment is due at the time of the session and processed via Sessions Health. Receipts are provided for insurance purposes. 

5. Confidentiality 

All communications are confidential and managed per our Privacy Policy. Confidentiality may be broken in cases of harm risk, abuse, or legal duty.
